Who’s at fault?

The local crew who left the switch open to the spur… It was their fault, plain and simple. That’s not to say that they were stupid and ignorant, they were most likely fatigued (one of the biggest safety issues in railroading) and just plain forgot. Not really to blame, as no one got hurt, and they just made a mistake and proved they were human. All humans make mistakes…just some have worse outcomes than others…

Are you going to tell me no one got fired or reprimand just because they forgot to turn back a switch? The company loss a lot of $$…Let me apply to work in that company. [X-)]

I didn’t see anyone say that, and I am sure someone got fired. Who depends on the circumstances. I am willing to bet that the crew of the local got run off, and it is quite possible the dispatcher got at least time off. Since this was dark territory, emergancy order 24 requires the crew to give the times the switch was reversed and returned to normal position to the dispatcher. If the times were not given, and the dispatcher allowed them to clear their permit, the dispatcher would most likely get time on the street as well.

Were you there to tell if they were fatigued?

Were you there to tell if it was a switch crew that left it open? Could it have been some kid playing with a switch that wasn’t locked appropriately? (Let’s face it, any career railroader can tell you about at least one lock they have found that doesn’t require a key to “open” )

Unless you’ve worked in the industry, you’ve never experienced what you’re talking about.

Emergency Order 24 has been rescinded.

SPAF is no longer required, unless the railroad chooses to continue using it.

We’ve been through this one before. For those of you who didn’t see the FRA accident report on the last go around, use this link to get to the PDF file. Since all three crew members were found to have violated EO24, all of their jobs were in jeopardy. Their omission occurred seven hours into their duty time.
