Generally on the Oregon Short Line (UP) in Idaho IIRC the milk car would be located between the engine and the RPO.

The book “Set Up Running” has an extended discussion of a PRR milk run. IIRC the milk was picked up directly behing the locomotive each of several times. That would be the simlpe and safest way to do it.

Al Johnson’s shots of C&NW trains 703 and 706 (Chicago-Freeport) from the 20s and 30s show the reefer on the rear of the train. Generally a two-three car consist, frequently pulled by an Atlantic.
