Two passenger trains collided Head-on in Southern Poland, 14 reported dead.
Head on - immediately brings into question what the rules of operation were for the territory involved?
Or rather, what form of protection they were using for the wrong-main running.

Sounds like the Rail Traffic Controller is being identified as the cause
