Sounds like a real thorough investigation of a set of circumstances which could never be anticipated. But you just can’t predict every possible sequence of events. The story doesn’t note any injuries or fatalities, which means the precautions in effect worked.

Speed restrictions for empty cars have been in effect since the 1980s in the USA for exactly the same reason. New and well maintained freight car trucks will not hunt, but it is difficult to determine if truck components are worn enough to be a hazard by visual inspection alone.
